No injuries , just held up the pour for a while as it blocked the only access I took that pic almost 10 years ago , wow does time fly!

That is one oogly mixer. I had one do that a couple of weeks ago. The excavator just convered a trench without compacting it and, didnt have it taped off. One of the worst job sites I have been on for awhile.
