Something sounds fishey here. What is the chances of getting our hand caught in a water box while pump is running. Why is the cover off to begain with?? Why is a operator near a water box while running a pump??? He should be near to the placment of concrete and looking at the end of the hose not at the pump unless he is having a problem, that's what it sounds like too me. Equipment has covers on it and safety signs keep hands out . Why was this cover off.

He probably was running a hard line and for some reason stumbled. IF he were trained properley he would have been on the ground, alot of companies run junk that should be tagged out and this is the result.
