A concrete pump truck was tasked to pour concrete at a compressor foundation. While the operator was lowering the fully extended 42 meter boom to the left side, the truck tilted. The rear-left side outrigger leaned into a cable trench wall and the fully extended boom fell onto the rebars installed over the area to be concreted. The right side outriggers were also lifted off the ground. It was observed that the rear-left side outrigger was not extended to safe position. No injury to personnel was sustained and the pump truck was re-positioned to a stable position.

Immediate Corrective Actions :

Outriggers of the concrete pump truck are to be fully extended according to the load chart when using in a blocked capacity. Use of suitable packing to spread the load was emphasized.
