Look how that machine was short rigged!!!!  You move the balance point so far back without opening up that front outrigger!!!

I'm kind of bummed there is no sound.  I'll bet there was a HUGE popping sound when the operator's head came flying out of his ass!!!

What a great learning tool this video is.

Thank goodness no injuries,that was the good.

This is exactly why the "Ricky Racers" of this industry should never be allowed to run a trailer pump,much less a big boom.So this is what a contractor gets for 1st pour free? Serves them right,that was a bonehead move on inexperiences part.They deserve everything they get!!!!

We have been looking at the video and it looked like to us that he put the outrigger in(or didn't put it out ?) to let that truck back up in that bay to the left of the pump. Not sure though. Regardless he made a  BIG mistake. Lucky nobody was hurt.

Amazing, 100% operator error.  Operator should have never went past extended rite front.  How many diagrams are these new guys going to look at before the material sinks in?  If a contractor has enough money to build a high-rise building he should be able to afford a permit to close the road.  I worked in NYC for some time and it was manditory to close the road and have a flagman.  Think thats expensive, see what it costs when the company pays the liability on involuntary manslaughter,  just like being in a car accident if you are drunk or impaired I recomend the operator has to fly to Atlanta for a month so Mr. Bob can teach this young man something about SAFETY!!!!!!!!!!

The Video should be put back up, no matter who asks, this is more important than a companies image, or a single individual's ego! THIS IS SAFETY! That takes priority over all else! 2cents!

That's ok Todd.Why would they worry about me in Colorado? I can't hurt them or there bad choice in operators.Everyone locally knew about it within hours,it's called self inflicted wounds.However if the owners are that concerned they can feel free to call or mail me with a explanation of why this happend.Oh,there's no sympathy here for them.

When i got the video i posted it, i thought it was a great video to show everyone for safety reasons.  I did not post it to give the owners a hard time. I was out when they called and when they called and left a message asking me nicely to take it down I did. I took it down out of curtsey. I hope to talk to them later on today if not I will just put it back up again. I hope to use the video as an example of what not to do. If the video is not online for a month or 6 its not that big of a deal.

Outside of America its of no importance who the pump company was/is, this is a great video of what not to do. Blank their name out or give them credit (prob. not!) or whatever because there arent many training videos like this about.
