That's Horrible!! I hope the injured make it. I want to see the front out rigger. It looks like he might be set up on some bad fill or something. the slope of the bank don't look bad enough to cause the pump to tip.

I was told there was a pipe buried under the left front. It collapsed and you see the outcome...

This info came from the man that was running the hose when it happened.

It is a terrible thing to happen and, I hope that those injured recover fully and, quickly.

But is it just me or is there no dunnage under any of the outriggers? I dont mean to be critical at this time but, I see none in any of the photos and, it is at times such as these that we need look at the whole setup. Some may consider me out of line but, I see it as a learning experience. Not putting dunnage under the outriggers on soil with that size of a machine is never an option to me but, I have been known to be wrong at times. Granted the photos are not that great and, I see one pad on the right rear but, on the right front I see nothing and, on the right rear. Even with my 32 on back filled or freshly excavated soils I like to put extra under my pouring side outriggers. Like I said I maybe wrong.

The one that northwest63 refer's to up in our neck of the woods happened because the operator was not told that there was ten feet of foam under grade. Prayers go out to the injured and family.

This was a very unfortunate ACCIDENT.  It would not have mattered how much dunnage was under the outrigger.  I ran a pump exactly like this one for over four years, and thats alot of weight.  If the operator would have been informed that there was an underground pipe, then this fellow certainly would have not set up on it.

There is only so much we can do as operators and I personally do not believe this was in any operator error.

The contractor should have known whats underground.  He has the blue prints of the entire job.  It just angers me to think because someone has to take the blame, and almost always its the poor operator trying to just make a living.

I agree with you bigstick.

Operator was told were to set up and the contractor should own this one.

I know State Wide and they are a great company and a good group of people.

his pads were out look on the concrete  the front right outrigger pad is sitting on it.if thi operator took time to put a pad on the concrete i think he would have put them everywhere right rear is on the ground frount left is one story down most likely. there are 3 pads maybe left rear sunk or maybe it was moved. I have worked with the statewide boys and they are a great bunch of guys. i think the contractor should have told them there was a buried pipe or have a crane mat there for the pump or something to dispurse the ground load of the outrigger.

do we all agree that this could be a customer liability issue or do you think skrewing up a half million dollar pump is what we sign on to do.

Now that's the pic I wanted to see,an insurance companies worst nightmare.I hope things work out for the injured and there families.

You gotta feel sorry for the operator and people that were hurt . It looks like everyones worst nightmare I dont think you really know how much pressure is under the outriggers until you have run a machine , a lot of contractors are too relaxed about it they dont understand the potential these machines have to do serious damage! Looks like the contractor wanted him there because its the best spot for max reach.. max reach isnt always the best option , i dont think tibers would have saved this poor guy!

pudg2 04-14-2010

I dont think a crane matt would have saved him there,so sad, this is what we deal with everyday its a guessing game whats underground ? all we can do is dunnage if it doesnt hold atleast you tried , these contractors have to be responsible for underground piping and such we are not equipped to xray ground conditions,or read minds ,someone knew that pipe was there ,theyre the ones at fault.I was setting up a 32m one day and the rear pad sunk a little bit when pressure was put on it, finished my setup and went to check it and put a little more downforce on it and crack and my pad is gone into a septic tank, if I wouldve unfolded and swung over it it wouldve for sure flipped, luck was with me that day.

My prayers go out to the injured. I don't see any donage under the outriggers. Would donage have helped I don't know. The sad story is that insurance company's and OSHA are going to ask the operator why no dunage. Also did they let the operator wash the pump out ?

Looking at the pics . The pump can't be washed out .

Some people shouldn't comment on things they have no idea about!! I work at state-wide, and bigstick you got it right :) Some people talk out of the wrong hole! Everyone that jumped to conclusions, think before you start making accusations!

b-alto 04-15-2010

Why couldn't you just let the pressure of the boom let the truck down and wash it out?

pumpjockey 04-15-2010
reply profile send pm notify

Showtime, 99.87% of the blame belongs with the general contractor. The operator is 0.13% to blame for trusting anyone. That being said, we all make lucky calls every day, this one caught up to him. Shit happens, luckily, no one died, as so often happens. Everyone should take his lesson an learn from it.

As in all fatalty accidents everything stays where it's at,car, truck, bus, allows investigators to do there job.

trench plates would have been nice!

pink panther 04-26-2010

That is very unfortunate. By looking at the pic, it looks like those are vents for the parking garage directly in front of the pump. Not the best place for outriggers. Hopefully this will help others to look out for things like this. You never know what lies beneath.
