I was asked by parties involved to take the photos down. We will now focus on boom inspections and repairs. Ken Williams Jr is working on setting up a charity for the Michel Pacheco. This should be set up next week.

I am happy to say that boom inspections are on the rise.

I can fully understand the position you were put in over that accident thread. Im sure the lawyers will take it from here,it is unfortunate but it always seems it takes an accident to get people to take notice the importance to safety issues in our industry. I was offline for the most part the last couple weeks and did not get much of a chance to respond to a lot of the questions that were brought up, but sometimes things are best left unsaid or typed in this case .

As far as concrete boom inspections go , I personally have seen an increase in inspections and my fellow inspectors in the field that I know also have. That is a good sign that companies are still maintaining there fleets to the safest degree they can.We still have a ways to go in the US market ,but it takes time for all owners of concrete booms to get on board for reliable boom inspection companies to assist them in the safe operation of there equipment. 

If any owner is contemplating a boom inspection in the future and you do not know which company is best suited for your brand of boom, please do not hesitate to directly contact the manufacture and get there input on who is certified to work on your equipment,they are very helpful in this particular subject.

Isn't there a freedom of press in the states. all what was showen on the pics was a broken leg. no names no brand???

Yes we have freedom of the press. I was not forced to take it down, i was asked and I did it just like I would if it was you or you company.  We still move forward and do the work. I have all the pictures and all the video still and someday I would hope we can post it all again soon.

  This accident has gotten a lot of attention from manufactures of concrete booms and the process in which Inspectors and authorized welders are involved.

  I kind a wish we could have the thread back to discuss it more in depth.

  Today as i was doing an inspection for a company , I was over come with anxiety that i did not want to miss any detail that could potentially cause an accident in the future,exspecially after Todd posted those injury picture's of that poor 23 year old fella and the life long struggle he will have after being crushed by that boom. And to think that something as simple as a proper Inspection may have prevented this tragedy made me stop and think twice about taking any short cuts during my inspection process today.

  My take on this subject is basically, there are no short cuts when it comes to the safe operation of heavy equipment.Your bottom line cannot afford a possibly preventable accident. I do not like the the expense nor inconvenience of a vehicle inspection that is mandatory in my state, but when my mechanic/inspector tells me i need new brakes I take his advice and put new brakes on and I think to myself he just may have saved me or my family from an accident. That in itself is worth the price i pay for his professional advice.

well said my friend! just this past week i found myself doing the same thing ! looking at the pump just that much harder! and all well knowing that i was doing my job right!!! but we as individuals have been trained to do this !!!!! never before in canada have  i had someone say to me no i dont need an inspection its not law or required absolutely not nessasarry but it has happened my eyes were wide open when the owner said it so!!! the legislation is out there people look at it!!!!!!! its your obligation! know where you stand people it is the law if you choose to break it the consequences are severe!!!!!! boominspector thanks for keeping the industry real!!!!!!

Its really not a brand issue, it looks to me it was a maintance or repair issue. Who knows.

Boom inspections? Why? The boom had nothing to do with this accident! It was the OUTRIGGER that failed, not the boom. I'm sticking by what I said the first time. The outriggers on these trucks need to be tougher then what the "Concord" concrete pumper trucks engineers think. However, they got the front outriggers right! Thick "I" beam steel. If you noticed in the picture... the front outriggers on the truck in question did and continued to support tremendous weight even when the truck was on it's side. Here is the pic just in case you missed it. I unloaded into this pump 20 minutes before this accident. The operator is a good operator and my friend. There is no way he could have known the outrigger was going to fail. He's also very upset about the incident. We all hope Michael is doing ok. It's very plain to me. The rear outriggers on this style of pump are simply not strong enough. A boom inspection does not entail just a look at the  boom, we inspect all the structurual steel on these trucks including the outriggers. Since there was an injury in this accident, Im not going to add any more to what I just said.(legal reasons)

No offence but if you don't even know and understand that a boominspection entails much more than just the boom. With other words: You don't know what a ''boominspection'' is... Than how can you think you have enough knowledge to explaining Concord how they should make their outriggers?

Sorry for your friend btw.. I don't mean to defend Concord or say they are fantastick or anything like that but just saing we should be a bit carefull about what we say when we actualy don't know what we are talking about.. It can couse dammage to a company that might not be ''deserved''
