Fully extended boom fell onto the rebars

What Hppened is..

A concrete pump truck was tasked to pour concrete at a compressor foundation. While the operator was lowering the fully extended 42 meter boom to the left side, the truck tilted. The rear-left side outrigger leaned into a cable trench wall and the fully extended boom fell onto the rebars installed over the area to be concreted. The right side outriggers were also lifted off the ground. It was observed that the rear-left side outrigger was not extended to safe position. No injury to personnel was sustained and the pump truck was re-positioned to a stable position.

Immediate Corrective Actions.

Outriggers of the concrete pump truck are to be fully extended according to the load chart when using in a blocked capacity. Use of suitable packing to spread the load was emphasized.

Under trained operator?

Trainer said you can do this?

Company policy,or lack of one?

No safety program?

It's sad inocent people are put in harms way.

Looks to me that the left rear outrigger isn't out at all. Looks like it is on dunnage right by the tires. Dont understand it!!!

Once again we see where there was no thought of safety first and, telling the contractor it isnt safe for me to setup here. I actually hate second guessing another operator but, this is clearly wrong from the get go. I see just one thing from the photo that could have been done and, that would have been angling th pump in to fold and, extend the rear outrigger fully.

That's old news. The back leg was short rigged even though he knew he was booming over that side. They were lucky enough to be able to reset the pump and finish the pour!

An operator that wasn't properly trained is not all the blame.Yes,some take to pumps/cranes like fish in water,some don't.I tend to look at the training,company safety policy,believe it or not just talking to other operators.

outriggers /turn signals just an option;it seems like more and more people are refuseing to useing these very important options' they think there in place to look at, or make it look a little better. NOT .believe it or not its there for a very good reason/yea thats right SAFETY.....

my main question would be, why was this machine upprighted and used to continue the pour. i would think it should have been removed from site and inspected. as for the set up i'm surprised that even a newby would fulrigg the offside and try and operate over a shortrigged side.
