Looks like He was done and moving out so they could tailgate reorder and pulled outriggers before boom was put away. |

Heard about this,and saw one of the operators the next day who was on the job. According to local ready mix gossip, there was a trainee involved and up to two operators on the job. Primary operator was in the process of folding up, and Trainee put legs in. How this can happen I don't know , If the remote is in boom mode on a putz you can,t move the legs/ I believe said pump is a schwing so don't know if you can do both. I wouldn,t think so. I repeat this is all readymix gossip so for what it's worth.... No one was hurt from what i was told/ Thank God. Pump safe Guy's.......Bobby. |

Don't believe its a schwing unless its one of their new z's. If it is, you cannot run outriggers when in boom mode. By the looks of the pic, no outriggers were even deployed at all. Laying to flat I would think. I don't know how they would get that boom to almost full stretch without rigs though. As long as everyone is ok, thats all that matters but I'd still like to know how it happened???????????????????? |

You would think there would be some kind of safety device that the outriggers our out only then can you move the boom. No outriggers no boom use. |

Looks like to me they were done pumping brought the boom tip to rear of pump suck sponge back. Then the real problem started.Maybe (operator trainer) said we got sponge back so HE going to the portalet or talk to the finsher or mybe to the job trailer to see when the next pour was. At this time is when things went wrong! Maybe the trainiee (rookie) unfolded the boom set up this pump before with supervision of an operator.This looks to be the first time he did it alone. He knew how to put the pump in outrigger mode.But he did not see the big picture THE BOOM IS UNFOLDED.It could of been because the operator was the one who brought the boom back to the rear of pump to suck a sponge.Then operator said you got it from here!THE ROUTINE was changed. I have been a concrete pump operator for 33yrs. And I have trained alot of operators . When I am training a soon to be operators I show them my routine and they hold the box the hole job Even if I have to operate pump (things not moving fast enough or smooth enough) I let the trainee bring the boom back to pump suck sponge fold boom bring in outrigger and wash out pump. I am right next to him. I help them create a routine. I hope no one got hurt I BLAME THE OPERATOR TRAINING THE TRAINEE Never leave your trainee alone untill he is ready to operate that pump by himself. I hope you guys feel the same as I do. PLEASE BE CAREFUL DON'T LET ANYTHING GET YOU OUT OF YOUR ROUTINE. May GOD bless all your days ahead |

This is obviously another case of sticking the boom straight up in the air to suck the sponge and putting the outriggers in with it still up... Pretty ridiculous if you ask me. I just cant figure out how in the hell someone can forget that they have ovdr 100 foot of boom in the air... |

The trainee is very lucky it didn't fall on him if he was standing at the side pulling the legs in! Looks like a Cifa 36z, would have been a nice pump! |

I know of several cases just like this. Operator gets out of his routine, or doesn't have one. Sucks in outriggers and pump falls over. I know of one 32Z that drove about 200' before it fell over. I was at the site and while I didn't see it fall I saw the recovery effort so I know it's true. |
