Here in Az. we had a pretty bad accident this morning. A crew was pumping a wall and the reports are that the hose exploded and blew concrete everywhere and injured 3 people. 2 of the had to be lifted off the wall with a crane. I will not name the pumping company because that is irrelevant, but this is a reminder how quick a day can turn very bad. BE SAFE |

PHOENIX -- Three people were injured in an accident at a water plant Friday morning in Mesa, fire crews said. Reports said the accident involved workers moving concrete slabs or a concrete pump explosion. One patient was stuck on top of a 25-foot wall, and fire technical rescue crews were working to remove the worker with a crane and Stokes basket, authorities said. He was airlifted along with another patient to Scottsdale Osborn Trauma Center. The two men going by helicopter are age 44 and 50, and they both have face, chest and abdominal injuries, scanner reports said |

I heard about this at noon and did not say anything about it. I always hate these things. I hope the guys will be ok. I hope there was no metal end on the hose. |

It would be very helpful if someone that knows what happened would explain it. The how - what - why of it. Then perhaps we can come up with a way to make sure that this never happens to you. Accidents can have value if you can learn from them. Otherwise they are 100% negative and will continue to happen. |

There was a simualar incident that happenend in our regoin just wensday. Hose whipping accident... the operator had 12 plus years experience and the hose man had to be air lifted from the job.. I don't have all the details as of yet .. but it shows that it can happen to anyone.. be safe everyone.. keep those injured in ur thoughts and prayers |

I will do some asking around but that is like pulling teeth sometimes. I am not looking to put anyone down. Just want details so we as an industry can do more training to keep these accidents to a minimum. They were pouring a wall, so they should of had a wall pipe on. So my guess as of now would be that they did not get hose whipped, but a plug that blew the hose apart. I am not sure but I believe they use a double ended 5" hose then the reducer, so it probably plugged in the reducer and the hose might have blown. This is just my guess, I will see what I can find out. |

5" boom pipe and 4" wall pipe calls for a 5-4 reducer in between. Most companies here want a 4" pipe. |

That specific company also uses a spade for pipes and they are dangerous if you don't know what your doing. |

Was this a flexible tremmee or a thin wall pipe? We've lost work to companies because we won't run the flexibles. They're basically water hose. |

From what I understand the wall pipe burst due to plugging, we use the water hose type wall thing, it has its advantages but it can make it a bit dangerous for the inexperienced. |

In an earlier discussion about this type of product I suggested that it would be much safer to use a protective cover over the hose. Someone else suggested a similar product of larger dimension be the cover. It drives me nuts to know that there is a simple easy way to do it safer and yet nothing is done. I hope that the people that were hurt are getting better. I wish we would DO BETTER |

Still not sure of exact details, but I was told 3 days ago that the thin wall pipe had plugged and then and blew the hose apart. Talked to one of the operators for that company today and he does not know anything because they are keeping it very quiet. The only thing he said is that it plugged and the pipe broke at the fitting where the clamp is. So, if that is what happened then, the crew could have gotten hose whipped when it came off of the wall pipe. Only a few people will ever know what really happened. Hope everyone is doing OK. |
