66 Year Old man was killed in Florida this morning during a pour when a 62M Boom snapped at the first section.

Heard who it was but and the manufacture but I think that is not important here. Rather give the wasted time in negative talk to a prayer for his family.

Please at least tell us what part of Florida it is I live there and would really like to know what redimix co. and what part of the state it is.  Prayers for the poor guys family.

Kendall, FL.  - C&C - 62M Sermac - Maybe a telescopic boom on that machine. Ready mix driver was killed instantly.

All I have guys. Sorry. I am sure it hit the news by now. Check on line for the local Florida news stations in that area. I have not had time to chase it all down.

1 Dead After Construction Accident

Worker Killed While Pouring Concrete

POSTED: Monday, April 27, 2009
UPDATED: 2:27 pm EDT April 27, 2009

One person is dead after a crane-like machine collapsed Monday morning at a construction site in Kendall.

The collapse happened at a construction site in the 7200 block of Kendall Drive.

Workers were pouring concrete as part of a re-waterproofing at the underground garage at the site. The crane hoisting the large hose through which the concrete was being pumped, also known as a boom, somehow gave way.

In the chaotic tumble of machinery, parts and debris, a 66-year-old longtime employee of the concrete pouring company was crushed by the end of the concrete pump, Local 10's Glenna Milberg reported.

Federal work safety investigators and Miami-Dade police homicide detectives are just beginning their probe into what happened.

The worker who died has not been identified.

We dont know what happen but what ever happened this hurts us all. So sorry for this families lose. This may not be Sermacs fauilt i saw on the video that the wind was so strong that it was setting off car alarms, you know it had to be mad windy for that to be happening.

It was the rumbling when the boom hit the ground that set off the car alarms for blocks. Not the wind.

Todd 04-27-2009
reply profile send pm notify

There are two videos posted and if you watch them they said that it was so windy that morning that car alarms were being set off. Now maybe your are right but that is not what she said.

tebequip 04-27-2009

Maybe both right. I got a call from the site and he said when the boom hit it was like a bomb going off. I asked about the alarms in the background and he said it was the boom falling, noise and vibration.

If you heard it on two reports then maybe it was both things. Like you said. Would take tremendous wind to get the car alarms going.

I really like the guys at C&C I hope it was not their fault or the operators fault, they are a really good company.

That is just terrible looking at the break it looks like it was cut with knife. I feel bad for the family of the worker and for the operator......

DON'T STAND UNDER THE FUCKING BOOM!!! WORST CASE SCENARIO RIGHT THERE! my condolences to that man's family and all involved. i wish we could stream this video when the crews give us funny looks when we tell them to get out from under the boom. it could be them not going home.
sorry about the language, but the situation calls for it...jjj

For once I agree with you JohnJohnJohn,  I tell people everyday the same thing.  You will never see an experienced pump operator stand under a boom of any size.  People have acres of space to stand but the all want to gather around the hose and under the boom.

Horrible news! Very sad... Sixty six years old? Poor fella was probably a short timer, counting his days til retirement? It's a cry'n shame!

I will excuse your language jjj. After reading about it, once I saw the pix & video I wondered why the heck a mixer driver was under the boom in the first place?

I feel just as bad for the operator, that kind of thing is hard to shake. He may never unfold another boom again and who could blame him? I am no softy but something like this sends my heart up into my throat. My thoughts are with the families of everyone involved...

Im not knowlegable about Sermac pumps,  so I cant say anything bad about them.  Hopefully it was not a manufacture defect,  or operator error.  I know the Manager for the company in Orlando I'll call him tomorrow or the next day after everything calms down,  lets all say a little prayer for the guys family.  Its always sad when this happens.

so sad to to see these pictures.  my prayer and thoughts to the family.

all that can be done now if for all us operators to keep on reminding people not to stand under the boom.  man that was a clean cut.  can happen to any boom to any operator.

a speacial prayer to our brother operator.  he's ganna need all the brothers and sisters support right now to get his next boom in the air.

Where are you Bob?  CP.com needs you...

Hello Tom,

Any idea for help to his family from all members of concretepumping.com

If any then I will be first ready for that.

Thanks

Doesn't it look like it broke off at the end of the telescopic section? That is a weird boom, seems like a awful lot of weight after the telescopic section. According to the spec's there is 150' of boom after the telescopic section. I am no engineer, but that seems weird to me.

FrankieW 04-28-2009

By "texas boom" do you mean a steel tremmie connected to the boom by a 5" rubber hose and supported by the crane to extend the boom a little further?

tebequip 04-28-2009
reply profile send pm notify

This telescopic Boom is an original design by Reich of Germany. IT is now used by Putzmeister, Sermac and others have booms on the board.

I was told by Reich that this is an odd place for a break on this boom. Bigstik may be correct that there were attachements on the end. I understood the same thing when I first got the news but like stick waiting on someone to come out and actually tell the story.

Whatever it was. A man lost his life and we need to learn from it. Hopefully we can find out the 100% correct answer on this failure and learn.

There are some pictures of it being constructed at the factory online...

Hey TT - Reich has had the teleboom on 44m pumps for several years now. They introduced this design in the late 90's with this. I'm with you. Tough to imagine that big boom with this option.

I did forget that Schwing also has it on the 31. Thanks for the reminder.

Sad news

But why do they put the telescopic on the main where the weight it at the max?
Tried a Schwing(I think...) 31 once with the telescopic on the 3. or 4. section.
Never got the idea of a telescopic boom, I would rather have an extra boom.

Bigstick, they were not using a texas crane. Not that I know of, miss information. I think we all need to stop speculating and wait and see what happen. It will be a while before we know. What we do know is that a poor guy died and this is bad for everyone. Lets let this sit for a bit.

pump n006

yah, another section is better

i'm not a fan of a telescopic on a pump boom.  i would much perfer a 4.4m 5th section  with 240 articulate vs 4.4m - in -- out.  wonder if the boom can be telescoped when fully streched with those bigboom?

Hy, im sorry for this man and his family. But i can say one thing this is a very good boom. In our company we have also a Sermac M62 Telescopic it is about 4 years and was pumping over 1million kubikmeters.

pudg 04-29-2009
reply profile send pm notify

I am no engineer , but just doesnt seem rite that big of a boom that telescopes , seems like a accident waiting to happen , and with all that weight stretched can it be telescoped with the boom out  or does it have to be done with it straight up and down do not know just asking ? my condolences to the man who lost his life and to the operator.

DIGGER 04-29-2009
reply profile send pm notify

It sure is so sad about that poor concrete worker.

From one of the video scenes, it appears that the first stage boom was pointing down hill. On the basis that it was, and the boom telescopes out 17 foot, COULD it be that whilst extending out and down hill with all the weight of the 3 other stages .... the sliding section kept going due to the holding valves not set correctly, if the valve held the only other stop would be the cylinder piston, if that pulls off the rod ........ you then launch a rocket.

Engineers in sheltered workshops ....... do not consider what a concrete pump operator can do with their designs. Not to blame an operator ....... it maybe have been an un-seen event, not covered in the operational manual of such a radical design.

Is there anyone on here tring to find out what really happened? We have 36 posts on here all speculatoin on it.Todd, Are you working on this?I don't want to be a ass, But we need to find the truth to this and try to fix it before someone else gets killed. It's been a week now and we all are guessing on what happened.Lets try to find the truth.

Just my thought

Just like any accident especially when there is a death nobody wants to say what happen. I wish we had a organization like BC that does a Accident Investigation and report like this one...

http://www.concretepumping.com/dictionary/index.php/Concrete_Pump_Accident_Investigation

Nobody is going to say what happen. I have heard all the rumors everything from extreme wind, Texas crane and now that the boom while telescoping out did not stop at the end were it should of and kept going until it just fell off.

Who knows what really happened but one thing for sure the mfg will be all over it. We might not ever know what really happen, or this might take years to find out.

Thanks Ruck

Being an operator is not a easy task but is key to safer operation.Part of my pre trip was to take a few moments to just slowly do a walk around looking at boom. Now this may seem strange to some, but I have seen failure. One never knows what the former operator did,so take the pesimistic approach. It only took a few minutes of time but I knew I did my part.

I agree the results should be made public, other than osha reports.Yes the manufacturer will be deeply involved so they can determine if modifications are required.

One can bet there last buck safety bulletines are going out. They will also be looking into "BOOM INSPECTION RECORDS".As Bob would say,did I mention "boom inspection records"
